Attn: Affiliate Manager 17410 NE 40th Place Redmond, WA 98052 The IRS requires us to submit a W-9 form for any individuals/sole proprietors paid over a certain amount. This is not required if you are outside the US or are a corporation, LLC, etc. If you have been requested to submit a W-9 or social security number and do not respond, we will withhold future affiliate payments until the information is submitted.
